Preparing for Your Oral Surgery Consultation

We look forward to meeting you. Preparing a few items before your consultation helps ensure your visit runs smoothly and allows our doctors to focus on your care and treatment options.

Please review the information below and bring the requested materials with you to your appointment.

•	Complete your online registration forms prior to your appointment so the doctor can see you at your scheduled time
•	Bring a list of all current medications
•	Bring both your dental and medical insurance cards
•	If X-rays were taken at another office, please have them emailed to info@pacificofs.com before your visit
•	Bring a copy of your referral, if available
•	Bring a parent, guardian, or decision-maker if treatment decisions may need to be made during the visit

Medications & Medical Conditions

Continue taking prescribed medications unless instructed otherwise by your physician or our office. Please inform our team of any medical conditions, allergies, or recent changes to your health.

Arrival Time

Please plan to arrive early to allow time for check-in.

Consultation

During your consultation, the doctor will review your medical history, examine the area of concern, review any imaging, and discuss recommended treatment options. You will have time to ask questions and review costs, scheduling, and next steps.

Financial & Insurance Information

Our team will review insurance benefits and discuss estimated costs during your visit. Payment options will be explained prior to scheduling treatment.
