Insurance and financial information at Pacific Oral & Facial Surgery Center Insurance & Patient Co-paymentsIf you have insurance, we will help you contact your insurance company to confirm your eligibility. In order to best assist you, it is important that you submit your dental and/or medical insurance information to us with your New Patient Forms at least two business days before your scheduled appointment. This allows us time to contact your insurance company before your appointment.Should you be diagnosed with treatment services, we can provide you with a detailed plan for your treatment, providing the portion you will be expected to pay at the time of service. As a courtesy to our patients, we will send all the necessary paperwork to your insurance company so that you can still get a quick reimbursement. Our insurance billing team will diligently follow up on your claims to ensure you receive the reimbursement you’re entitled to. The insurance benefits will be based on the out-of-network benefit of your plan. Professional care is provided to you and our patients' families and not to an insurance company. The insurance company is responsible only to the patient as a benefit from the individual or employer plan purchase, and the patient is responsible to the doctor. We will assist you in filing your dental claim and handling insurance questions from our office on your behalf.
